Significant events in neuro-ophthalmology

Significant events relating to the operation of the service such as misdiagnosis or delayed follow up of a patient with a neuro-ophthalmological disorder should be reported via local reporting mechanisms and discussed at appraisal.

It is also appropriate to discuss significant complications of treatment, or severe visual loss due to a neuro-ophthalmological disorder (for instance a late presentation of giant cell arteritis leading to bilateral blindness). These occurrences do not necessarily imply any failing in the standard of care, but can help to inform future treatment.
