Eye Logbook

Online surgical logbook to record and analyse ophthalmic surgical data.

The Eye Logbook captures surgical data required to obtain the Certificate of Completion of Training (CCT). Information is displayed in both tabular and graphical forms, allowing the analysis of data for training progress, audit and revalidation purposes.

Resident doctors are mandated to use the Eye Logbook to record surgical procedures during their ophthalmology training.  They are expected to record all surgical experience, including procedures they have performed supervised or assisted at – please see letter in the Resources section.

The surgical logbook enables cataract surgeons to record expected outcomes of cataract surgery as S/CxA in addition to the SE.  Please click here for further guidance.

All ophthalmologists are encouraged to use the Eye Logbook, which is offered to College Fellows and Members in good standing as part of the package of membership benefits.  If you meet this criteria, please  contact [email protected] to gain access.

An alternative Excel template for non-members is available to download free of charge. Please click here for details.

NOTE: The Eye Logbook app on the Apple Store is no longer updated or supported by the provider, although it continues to be downloadable and accessible. Please contact the provider directly for details.
